On January 22nd 2008 Hazel galloway was murdered by her boyfriend Andrew Grundy.
After a 5 day trial at Exeter Crown Court he was convicted of Murder and sentenced to life imprisonment with a minimum tariff of 13 years.

Hazel was under the care of Devon Partnership NHS trust along with her murderer. Risks were identified but not acted upon.

There has now been a review into the care Hazel received by the Devon Partnership NHS Trust and further action is pending.
